AZV (Algemene Ziektekosten Verzekering) is Aruba's mandatory public health insurance scheme. If you live and work legally in Aruba, you are required to participate. Understanding how it works will save you time, money, and confusion at the doctor's office.
What is AZV?
AZV is a social health insurance fund administered by the Aruban government. It covers the majority of medical costs for insured residents, including GP visits, specialist care, hospital treatment, and most prescription medications.
The scheme is funded through income-based premiums paid by both employees and employers. Self-employed individuals pay both the employee and employer share.
Who is eligible?
All residents with a valid residence permit who are employed or self-employed in Aruba are required to register for AZV. Dependents (spouse and children under 18) are covered under the primary insured person's policy at no extra cost.
Visitors and tourists are not eligible for AZV and should arrange private travel insurance before arriving.
How much does it cost?
The AZV premium is calculated as a percentage of gross salary. As of 2026, employees pay approximately 2.6% of gross salary, and employers contribute an additional 8.9%. The combined rate is roughly 11.5% of gross salary, split between employer and employee.
There is a maximum insurable salary cap. Income above this cap is not subject to AZV premiums, and benefits are calculated based on the capped amount.
What is covered?
AZV covers GP consultations, specialist referrals, hospital admissions, surgery, maternity care, most prescription medications, physiotherapy (with referral), and dental care for children under 18.
Not covered: cosmetic procedures, most adult dental work beyond basic extractions, glasses and contact lenses (partially covered for children), and treatments abroad unless pre-approved by AZV.
How to register
Registration is typically handled by your employer when you start a new job. If you are self-employed, you must register directly with AZV. Bring your residence permit, passport, and proof of employment or business registration.
AZV's office is located in Oranjestad. Processing takes 2–4 weeks, after which you receive an AZV card that you present at every medical appointment.
Using AZV in practice
Always start with a GP (huisarts). Specialists require a referral from your GP — going directly to a specialist without a referral means AZV will not cover the cost. Keep your AZV card with you at all times.
For medications, take your prescription to an AZV-registered pharmacy. Most common medications are dispensed at little or no cost. Non-formulary medications may require prior approval or carry a co-payment.
